i think this should me the format when your are applying for mod/admin Smile

2. Why do you want to be a mod/admin? (State Which) -
3. What have you done on Disturbed to make it better? -
4. What can you do with mod/admin powers? -
5. Have you ever been Mod or Admin on any other servers? -
6. Who would recommend you to become a mod/admin , post their name here.
7. Have you ever broken a rule?
8. If you have state it here (or skip step if you haven't).
9. How many hours are you on a day? -
11. Will you be active on my forums ? -
12. Do you own a server or owned one? -
13. How old are you? I am 15 years old.
14. Why do you believe you should be Mod/admin?

Thank's for it, look's really nice im not gunna lie but, we already have one, butt it's not up to me.( if it was i'd use it)
another thing is admin's get picked my GA+

Very Nice suggestion, but we've already got an application formation
and btw my format is pretty much the same as yours, just less questions
but thank you for your suggestion.
